Step 1
Start a new document of 800 x 600 with white background and 72 dpi. Now do some text like i have below.

Step 2
Now create a new layer and grab the rounded rectangle tool and set the radius to 15px and draw a rectangle like shown below.

Now create new layer again and do some text you want to go in the box. tive angled it but you do what you want. to angle it press CTRL and T and swing it round.

now on the layer box shown below press and keep your fingure on CTRL button and click on the box its self on the text layer you have just created which mine is UBL.

Now let go of the CTRL button and click the layer 2 and then press the delete on your keypad…. once you have done that delete the text layer which mine is called UBL and you will have something like whats below.

Step 3
now in the layer box select all layers apart from the background and drag them down to the create a new layer. then goto Layor/Merge Down. Now goto Edit/Transform/Flip Vertical and put is below like the image below.

Now let make it a little more like its on the floor by pressing CTRL and T and then right click and press skew and drag to the left. once you have done that in the layer box click the add layer mask and press D on your pad. Press G which is your gradient tool and making sure that it is on black to transparent if its not make it so by going to the top left and making it so. now drag from button of the image to just above the top of the image. You should have something like this.

No to make it a little better press CTRL and T again and move the bottom up a little like below.

Step 4
Create a new layer and press M on your keypad which should give you the rectangle marquee tool. now do as image below shows you.

Now press B which is the brush and select the soft round 200px brush and make sure the opacity is set to 20% and set your brush to black and draw along the bottom till you get an image like below.

Final Step
Now press CTRL and D to release the marquee. Set the layer opacity to 50%. Now drag that layer down to the add new layer and goto Edit/Transform/Flip Vertical and drag it down and set the layer opacity to 60% and you have something like this
